He is survived by: his wife Teresa; his children, Eric Chavez of San Diego, CA, Erin Surratt (Brian) of Kansas City, MO and Angie Hoffman (Byron) of Ozark, MO; his brother Larry Chavez (Carol) of Darby, MT; his grandsons, Lewis and Carter Surratt of Atlanta, GA; and his pets, Tita and Coco.
