She was predeceased by: her parents, Hattie Belle and Thomas Underwood; her husband Buddy Dean Hazlett; her son Steven Mitchell Hazlett; and her sisters, Auline Harris and Marie Doerhoff.
She is survived by: her sons, Roger Hazlett (Debbie) and Harold Dean Hazlett; her daughter-in-law Cindy Hazlett; her grandchildren, Adam Hazlett (Amanda), Jessi Scholes (Sean), Jenny Hazlett, Dean Hazlett (Brittany), Roman Allen (Judy), Hayley Amelung (David) and Reston Allen; her great grandchildren, Andrew, Jacob, Josh, Amaya, Jackson, Liberty, Riley, Alyssa, Aiden, Cici, Avery, Liam, Dayton, Kaleb and Addison; and her sister Myrtle Martin. She is also survived by many nieces and nephews.
In lieu of flowers, the family requests that donations be made to Dry Fork Baptist Church in Crystal and Bud's honor.
