He is survived by: his wife Kathy Annette Cook; his children, Cristin Berendzen (Mandy) and Kyle Berendzen (Angie); his sisters, Cathy Klindt (Freddie), Brenda Rush (John) and Sheri Koestner (Todd); his grandchildren, Cayla, Lily, Corbin and Ellie; and his great grandchild Julianna.
