He was predeceased by: his parents, Robert B. Lewis and Ader Lewis (Spalding). He is survived by: his wife Betty Kay White; and his children, David Glen Lewis (Betty), Melody Trader (Ed), Cindy Bentley (Jack) and Rhonda Earhart (Mike). He is also survived by his numerous grandchildren and great-grandchildren as well as extended family members and friends.
