He was predeceased by: his parents, Robert A. Cook and Fern I. Woods Cook; his wife Louise Mae Thompson; his brother Robert P. "Bobby" Cook; and his parents-in-law, Louis Karl and K. Lucille Thompson.
He is survived by: his children, Iona Reed (Cody Sterling) of Taylorsville, Utah and Dale Cook of Phillipsburg, MO; his grandsons, Levi and Ezra; his siblings, James "Jim" Cook (Helen) of Lebanon, MO, Jack Cook (Karen) of Lebanon, MO, Terry Cook (Donna) of Lebanon, MO and Peggy Bohannon (Elvin) of Conway, MO; his brother-in-law Richard Thompson of Canada; Sharon Cortes (Tom) of Greenville, TX. He is also survived by several nieces and nephews; other relatives and friends.
