He is survived by: his siblings, Cynthia Millentree of Lexington, MO and Larry Fields (Laurie) of Phoenix, Arizona; and his nieces and nephews, Christopher Millentree, Cameron Fields, Jennifer Morgan, Stacy Johnson, Katie Townsend and Amy Fields. He is also survived by many relatives and friends.
In lieu of flowers, memorials are suggested to the National Kidney Foundation.
