Polen; her daughter Cusandra; her grandchildren, Nancy, Ted Jr., Becky and Raegan; her great grandchildren, Kelsey, Kaycie, Logan, Peyton, Lola, Ryan, Olivia and Beckett; and her great great grandchildren, Kypton, Gatlynn, Kolson and Grayden.

Donations can be made to St. Jude's Children's Hospital in her name in lieu of flowers and gifts.
