He is survived by: his wife Mary Mattox; his sons, Andrew (Chasity) and John Jr.; his grandchildren, Hailey, Mia, Maggie, Cayden, Anna, Cora and Carlee; his great grandchildren, Aliza and Rylee; and his siblings, Mary, Larry, Sharon, Theresa, Peggy, Annette and Lisa.
