He was predeceased by: his siblings, Charles Thomas Dunlap and Mildred Louise Klein. He is survived by: his children, Carson Wade, Lori Carol and Jason Max. He is also survived by several nieces and nephews.
In lieu of flowers, the family suggests memorial contributions to Bairdstown Cemetery or the Sullivan County Memorial Hospital Foundation.
