He is survived by: his wife Lisa; his children, Tonya Choate-McCall, Mitchell Choate (Lauren) and Levi Smith (Veronica); his siblings, Patsy Lee (Thurman), Martha Rader (Mike), Dan Choate (Debbie) and Jack Choate (Anna); and his grandchildren, Molly, Mason McCall, Cole, Anson, Raylin Choate, Ethan, Levi Jr. and Eli Smith.
