He was predeceased by: his parents, Gene Cook and Dorothy Cook (Jacobs). He is survived by: his wife Laura Gooden; his children, Joey Kaderly (Samantha), Joni O'Banion (Shawn) of Montevallo and Jerod Kelley (Cheryl); and his siblings, Karin LoFaso (Sal) of Indiana, Karmen D'Antonio (Daryl) of Independence, MO and Kerstan Cook (Christy) of Kansas City, MO. He is also survived by numerous grandchildren and great-grandchildren; as well as numerous nieces and nephews.
