He is survived by: his wife Amber Bourek Slater; his parents, Don Slater and Warlinde Slater; his sisters, Aunalise Johnson (Drew) and Danielle Daee (Brett); his sister-in-law Stephanie Bourek-Hoyt (John); his siblings' children, Aiden, Donovan, Layla, Brooks and Brock; his parents-in-law, Kathy Bourek and Darrel Bourek; and his pet Cooper.
