He was predeceased by: his father Homer L. Cook; and his son Justin Cook. He is survived by: his mother Edith I. Cook (Maxwell); his son Nathan Cook; his grandson Oliver; and his siblings, Sherri Stell (Cook), Beth Hutchens (Cook) and Jeffrey Cook. He is also survived by several nieces and nephews.
