He was predeceased by: his daughter Faith Fagan. He is survived by: his children, Cara, Annabelle and Daniel; his parents, Sean Fagan and Karen Fagan; his brothers, Joe Fagan (Krista) and John Fagan (Nicole); Shannon Fagan. He is also survived by the mother of his children and many extended family members and loyal friends.
Memorial contributions have been suggested to Sean Fagan, for a trust established for Dan's children.
