He was predeceased by: his wife Imogene Davis (Mayfield); his parents, Urie Marshall Davis and Ruth Elizabeth Davis (Bryars); his sister Virginia Bradley (Davis); his brother-in-law Rev. Clyde Bradley; and his son-in-law Rodger Carlson.
He is survived by: his children, Ron Davis (Judy), Sheila Carlson and Leah Hite (Dan). He is also survived by 10 grandchildren, 25 great grandchildren, and 2 great-great grandchildren.
Memorials may be made in Jack's name to a charity of donor's choice.
