He was predeceased by: his parents, Walter E. Sammelmann, II and Melba V. Sammelmann; and his sisters, Bonnie Hoester (late Bruce) and Ginny Conoyer (late Jim). He is survived by: his wife Barbara Sammelmann; his sons, Keith Sammelmann (Vickie), Rick Sammelmann (Kelley Ketterman), Mark Sammelmann and Brian Sammelmann (Christine); his grandchildren, Jay Sammelmann (Angie), Luke Sammelmann, Lexie Sammelmann, Erin Wurst (Jack) and Adam Sammelmann; and his sister Debbie Schneider (Tim).
Memorials may be made in Jack's name to Alzheimer's Association or to Masses in his memory.
