He was predeceased by: his brother Alex; his grandparents, Jim, Thelma Roberts, Dale and Ginger Book; and his uncle Jeffery Ammon.
He is survived by: his parents, Dave Book and Janet Book of Oregon; his aunts and uncles, Ken (Donna Roberts), Carolyn Ammon, Tom (Julie Book), Gary (Kim Roberts) and Anna (Brett Derr); and his cousins, Jane Roberts, Baylor, Noland Ammon, Mike, Madison DeSantis (Book), Guy Book, Ali Roberts, Emmi, Ayci Ammon, Millie and Thomas Book.
