He was predeceased by: his parents, Casper Lewis and Stella Hendrickson (Quam); his brother Lauren; and his grandson Joseph Lockert. He is survived by: his wife Anita (Fouts); his children, Mark (Mindy) of Waterloo, Stuart (Jaci) of Crestwood and Sarah Lockert (Mike) of Kansas City, Missouri; his grandchildren, Olivia Campbell (Casson) of Sioux Falls, Nick Lockert of Tampa, Paige Hendrickson of North Liberty, Mattie Hendrickson of Waterloo, Luke and Auggie Hendrickson of Crestwood, Missouri; and his great grandchild Laura Anita Campbell of Sioux Falls.

The family suggests contributions to the charity of the donor's choice in David's name.
