He was predeceased by: his father Bennie Hayward. He is survived by: his mother Dolores Hayward (Montemayor); his children, Ashley Dahms (Gregory), Heaven Hayward, Brandon Hayward (Matthew), Brittany Poland and Mercedes Hayward (Anthony); his stepdaughters, Jackie Poland and Kristie Auten (Jason); his grandchildren, Preston, Graesyn, Stetson, Fable, Mylee and Eri; and his siblings, Tina DeHaven (Kevin), Andi Hayward and Nick Hayward. He is also survived by other extended family members and friends.
