He was predeceased by: his parents, John G. Tevlin and Dorothy Tevlin. He is survived by: his wife Mary Catherine Tevlin; his sons, Daniel (Kelly), John and Michael; and his siblings, Michael and Mary.

“To Mary, John & Mikey: We'll always remember Jack as a most dedicated baseball coach ever... having a 'ball' with our team for 14 seasons (over 7 years). He was one-of-a-kind who had as much fun as & with us all. "We're huntin' for Bear" LOL. He helped coach us to that 1st win vs. the Bears & cost me big dough taking the whole team to pizza followed by a Cardinal game!!! I was looking for my good friend from long ago on Facebook, hoping for a reunion dinner when I saw the news. Great memories!”
