He is survived by: his parents, Sherry Maples and Steve Maples; his children, Houston Jerette, Kensley Sheridan Kay and Dacoda Olivia Lin; the mother of his children Hannah Kelley; his sisters, Amber Hainline and Erica; his nieces, Milana and Mei Lin Hainline; and his grandparents, Kenny "Ted" and Carol Robinette.

Since flowers wither and fade and enjoyment is brief, it was asked that contributions be made to Victor Baptist Children's Church- the church where Cody grew up learning about Jesus and where he was a member.
