He was predeceased by: his parents, Melvin Millstead and Marcelle Millstead; and his sister Suzy Trotter. He is survived by: his wife Kathy; and his daughters, Shani Harkin (Jeff) of Lawrence, KS and Amanda Steimel (Rick) of Marshfield, MO..
In lieu of flowers, the family has requested donations be made in Dave's memory to the Humane Society of Southwest Missouri or the American Diabetes Association of Missouri.
