He was predeceased by: his parent Nikki Polak (Bray); his former wife Eileen Polak; and his grandparents, Gene of Chicago, Nora Polak of Chicago, Rose Bray of Chicago and Bill Murphy of Williamstown.
He is survived by: his father Richard L. Polak (Patty) of Williamstown; his children, Kaitlyn Polak (Cody Davis) of Canton and Richie Polak of Williamstown; his siblings, Rich Polak, Jr. (Sarah) of Quincy and Marci Fitzpatrick (Ben) of Centralia; his siblings' children, Joey Polak (Bailey) of Lake St, Drezden, Milo Fitzpatrick of Centralia, Casey Duval (Steven) of Westlake and Jennie Polak of Boynton Beach; and his grandmother Patsy Murphy of Williamstown.
In Lieu of flowers, memorials may be made to the Providence Cemetery or towards Richie's Educational Fund.
