He is survived by: his stepmother Margaret Hendrik; his children, John Michael Hendrik, Anna Hendrik (Justin) and April Hendrik (Brooke); his sisters, Kim Hendrik and Kelly Jernigan (Tim); his uncle Jimmy Hendrik; and his close friends, Jeff Brown, Donna Ray and Chris Sawyer. He is also survived by nieces and nephews.
