He was predeceased by: his parents, Arthur Carmack and Janette Thompson; his daughter Anna Marie Carmack; his brother Gary Carmack; and his stepfather J. R..
He is survived by: his wife Debbie Carmack; his siblings, Judy Cox (Bradford Cox), Linda Evans (Tim Evans), Jack Carmack and Junior Carmack (Lisa Carmack); his sons, Jacob Carmack and Tony Carmack; his grandchildren, JoJo Carmack, Keith Carmack, Jayse Davenport and Heaven Davenport; and his daughters-in-law, Jennifer Carmack and Brittany Chalker.
