He is survived by: his children, Jerry Chadwick Jr. (Michelle) of Blue Springs and Jennifer Mathis (Richard) of Blue Springs; his grandsons, Trey, Tyler and Toby (Abby); and his sister Joy Lucas. He is also survived by several nieces and nephews.
