He is survived by: his mother Vera Broome Polk; his wife Brenda Polk; his children, Kristen Polk and Ryan Polk (Lindsey); his adoptive son Jazz Harmond; his siblings, Paulette Ainsworth and Jackie Polk; his sister-in-law Mildred Polk; and his grandchildren, Sarah Harmond, Alex Harmond, Seth Polk and Evan Polk.
