She was predeceased by : her parents, Jerry Wayne Polk and Mary Ann Dennington; her grandmother Grace D. Hubbard; and her sister Christie Adams. She is survived by : her husband Jimmy Smith; her children, Kevin Smith (Misty) and Colby Blackwell (Paul); her grandchildren, Micheal Smith, Justin Smith, James Carter, Braxston Blackwell, Bryce Blackwell and Jayson Smith; and her siblings, Jackie Polk, Leslie Thompson and Troy Snider. She is also survived by nieces, nephews, friends.
