He is survived by: his wife Charlotte Bolivar Madison; his son Stephen Bolivar (Susan); and his grandchildren, Carter Campbell and Katie Campbell. He is also survived by nieces and nephew.

In lieu of flowers the family has asked for donations be made to St. Jude Research Children's Hospital.
