He was predeceased by: his parents, Milton Polk and Myrta Polk of Prentiss, MS.. He is survived by: his wife Jan Barrett Polk of Jackson, MS.; his children, Brian Barrett Polk (Farrah) of McKinney, TX and Julie Polk Breazeale (Eddy) of Hattiesburg, MS; his brother James Sheldon Polk (Jan) of Southgate, KY.; and his grandchildren, Barrett Allen Breazeale, Blake Edward Breazeale and Olivia Kate Breazeale.
