He was predeceased by: his parents, Johnnie Slay Sr. and Phyllis Havens; his siblings, Laurie Slay and Kenneth Slay; and his son Johnny Matthews.
He is survived by: his wife Sarah Slay; his children, Ashley Slay, Stephanie Slay, Thomas Blair (Angela), Kristen Blackburn, Spencer Blair, Angel Slay (Zachary) and Madeline Cochran (Trevor); and his brother Darrie Slay (Manda). He is also survived by 19 grandchildren, numerous other relatives and friends.
