He was predeceased by : his parents, Van Otis Sullivan Sr. and Mary Lucille Sellers Sullivan; and his siblings, Jimmy Sullivan, Ricky Sullivan, Freddie Mack Sullivan, J. C. Sullivan, Thomas Sullivan, Dorothy Lucille Roberts, Mary Lou Sullivan and Janette Sullivan.
He is survived by : his wife Pamela Sullivan; his children, Vernon Sullivan Jr., Donald Sullivan, Johnny Christopher Sullivan (Beth), Ronnie Sullivan (Kim) and Kimberly Renee Sullivan Parkman (Justin); and his siblings, Van Sullivan Jr. (Margaret), Christine Gaspard (H. J.) and Mary Evelyn Smith (Jesse). He is also survived by ten grandchildren.
