He was predeceased by: his parents, Louis Costello and Mauvoline Costello; and his brother Eddie Costello. He is survived by: his wife Donna Costello; his daughter Tina Neely (Jason); his grandchildren, Joe Pribelsky (Ryan), Tyler Sloan (Katie), Mitch Fletcher (Franki), Danielle Mullins (Brandon), Dana Elliott (Isaiah), Tommy Costello (Lauren), Daniel Smith, Garret Studard and Dalton Studard; his great grandchildren, Zayne Elliott and Colton Sloan; and his sister Antonetta Medlin.
The Costello family requests memorials be made as donations to St. Jude Children's Hospital or to The Shriner's Hospital in lieu of flowers.
