He was predeceased by : his father Charlie Ballard; and his grandparents, Annie Mae, J. H. Ballard, Ina Mae and Dennis Baker.

He is survived by : his mother Barbara Ballard; his sisters, Regina Shaw (Marty) and Sheryl Patterson (Michael); his nephew Brandon Shaw; his nieces, Brooke Quick, Kara Patterson, Aubree Patterson, Whitney Jones and Regan Shaw; his great-nieces and great-nephews, Brailyn, Harper, Rip, Lexi, Laela, Mason and Marlee; and his close friend Debbie Applewhite Mixon.
