He is survived by: his wife Gina Herrington; his daughter Hanna Mayfield; his parents, John Mayfield, and Kathy Mayfield, Jr.; his siblings, John Mayfield, III, Cynthia Mayfield, Stephanie Mayfield, Joel Mayfield (Tally), Anthony Mayfield, Kenny Davis and Brandon Creel; and his close friends, Melvin Hesler, Josh Vanderford and Scott Broussard. He is also survived by numerous aunts, uncles, nieces and nephews.
