He was predeceased by: his parents, James Dvorak and Lydia Dvorak. He is survived by: his wife Rita; his children, Theresa Booke (Todd), Marlena Dvorak (Lisa), Bethany Moore (Josh), Randi Sanderlin (Dave) and Eddy Dvorak (Kim); and his siblings, Betty Tempero (Clair), Norman Dvorak (Margaret), Ken Dvorak (Val) and Tom Dvorak (Bonnie). He is also survived by 9 grandchildren.
