He was predeceased by: his parents, Goldie and Alfred; and his siblings, Eunice Kay, Cheryl and Gary.

He is survived by: his wife Carolyn; his children, Josh Cook (Natalie) and Misty Alexa (Kurt); his grandsons, Ty and Jake Cook; and his sister Diana. He is also survived by many nieces and nephews.
