He was predeceased by: his parents, William Cook and Louise Perkins Cook. He is survived by: his wife Dianne Bradshaw Cook; his sister Mary Louise Cook Rees; his daughters, Michelle "Shelley" C. Abernethy and Susan C. Williams (Steve Williams); his grandchildren, Tanner Abernethy, Audrey Abernethy, Hunter Williams, Gavin Williams and Hannah Cook; and his brothers, William "Bill" Lee Cook II, Charles L. Cook and Daniel L. Cook.
In lieu of flowers, the family requests donations to the Robin Johnson House of Dallas and the Crohns & Colitis Foundation.
