He was predeceased by: his parents, Charles Forman and Ardith Forman. He is survived by: his children, Joseph Forman, Jonathan Forman and Katie Forman (Meredith); his grandchildren, Luke and Landry; his siblings, Charles Forman (Gayle) and Cindy Sears (Ron); and his former wife Cathy Forman. He is also survived by two nephews and one niece.
In lieu of flowers, memorials may be made to a charity of choice.
