Jack will be remembered for the time spent with loved ones and the moments that brought joy to those around him. He is survived by: his former wife Helen Kruse; his wife Judy Surfus; and his daughters, Christy Bollier (Gary) of Wheaton, IL and Toni Harris (Rob) of Ft. Wayne. He is also survived by 5 grandchildren, and 18 great grandchildren, family members, are Judy's 3 children, their spouses, and 7 grandchildren.
