He is survived by: his parents, Jacob Ramsey Fultz and Casandra Nicole Black; his grandparents, Craig, Jenny Fultz and Carla Black; his great-grandparents, Jeannine Fultz, Virginia Ramsey and Polly Anderson; his aunts, Erica Fultz and Brooke Black; and his uncle Marty Black.
In lieu of flowers the family asks that donations be made in Sebastian's name to the American SIDS Institute.
