He was predeceased by: his parents, William Charles Booth and Pauline A. Mercer Booth; and his wife Patricia A. Smith. He is survived by: his children, Pamela B. Davis (Edward Davis) of Raleigh, N. C. and Samuel M. Booth Jr. (Roslyn P. Booth) of Clemmons, N. C.; his grandchildren, Samantha R. Booth, Taylor B. Davis, Sarah C. Davis and Ashlyn R. Chillemi (Jesse Chillemi); and his great grandchildren, Wynter R. Chillemi, Saylor R. Chillemi and Jayce J. Chillemi.
In lieu of flowers, memorials may be made to the Salvation Army.
