He was predeceased by: his parents, Lance Lee Cook and Nell Ingram Cook; his son Mark Lee Cook; and his siblings, Clarice, Luther and Bill.
He is survived by: his wife Elwanda Carpenter Cook; his sons, Jeff Cook (Dana) and Keith Cook (Marcie); his grandchildren, Lana Gibson (Edward), Zach Cook and Josh Cook; his great grandchild Liam Gibson; and his sister Mary. He is also survived by numerous nieces and nephews.
