He was predeceased by: his son Benjamin Horn. He is survived by: his wife Kelly; his father Willie; his children, Levi Horn (Amanda) of Rota Spain, Jordan Horn (Brittney) of Concord, NC, John L. Telek (Katie) of Louisville, KY and Kalyn Telek of Richmond, KY; his siblings, Steve Horn (Betty), Rickey Horn (Tina), Scott Horn (Valta) of St. Joseph, MO, Kathy Warner and Tabitha Smyly (Brad); his grandchildren, Savannah Horn, Lilly Kate Horn, Graham Horn and Alyssa Horn; and his mother Betty. He is also survived by several nieces and nephews.
