He was predeceased by : his father L. B. Cook; his son Bryan Cook; and his siblings, Alvin Cook, William Cook and Jackie Renee Andrews.

He is survived by : his wife Valerie Cook of Erwin; his grandsons, Jaxon and Noah Cook; his brothers, Larry Cook (Beverly) and Willis Cook; his sister-in-law Shelia Snyder; and also Amber Chambers. He is also survived by several nieces and nephews.
