He was predeceased by: his parents, Jake Smith and Aileen Smith; his daughter Debbie Boyko; and his sister Carolyn Carr. He is survived by: his sons, Michael Smith and David Smith; and his siblings, Elizabeth S. Wingfield and Tony Smith (Lou Ellen).
In lieu of flowers, memorials may be directed to Kitty Askins Hospice Center or the American Cancer Society.
