He was predeceased by: his parents, Frederick Carty and Martha Carty; and his siblings, Joan, Helen, Frederick Jr., Robert and Kenneth.
He is survived by: his wife Patricia Clementz; his children, Jean Cozzens (Jeff), John Carty (Anne) and Scott Carty; his grandchildren, Ryan Carty (Lalaki), Kelly Zimmerer (Greg), Kevin Carty (Annamarie), Alison Carty and Megan Carty; and his great grandchildren, Henry and Johyam.
Memorials or donations in honor of Jack may be made to the Hospice of Piedmont in High Point, North Carolina.
