He was predeceased by: his parents, Herbert D. Wells and Martha Plott; his wife Monica Lucey; and his pet Molly. He is survived by: his niece Glennda Wall; his girlfriend Ann Crutchley; his daughter Annabella Crutchley; his friends, Tracey Ridings, Zach Ridings and Sierra Ridings; his grandchildren, Madison Fulk, Aubree Ridings, Grayson Ridings and Everly Harrelson; and his pets, Frankie, Loki and Bleu.
In lieu of flowers, the family kindly requests donations to be made to a charity of the donor's choosing in Phil's name.
