He was predeceased by: his parents, George Andrew Cook and Julia Christine Rea Cook; and his sisters, Elaine Cook Manaugh, Jane Cook Long and Betty Cook Clayton.

He is survived by: his wife Diane Shafer Cook; his children, Shari Cook, Lisa Cook Born (Michael Born) and Thomas Cook II (TJ); his grandchildren, Stephanie Born Harrigan, Brianna Born, Tara Born, Maria Cook Chandler (Preston Chandler), Julia Cook (Logan Dehart) and Jackie Cook; and his great grandchildren, Nora Chandler, Emily Chandler, Oliver Lloyd, Lucy Lloyd and Sophia Harrigan.
