He was predeceased by: his parents, Summie Clarence and Minnie Costner Cook; his wife Charlotte L. Cook; and his brothers, Jamie, Alfred and Earl.

He is survived by: his sister Ruth; his children, Gary Cook (Jan) and Vickie Rhoades; his grandchildren, Kerry Penley (Kristan), Chris Penley (Beth), Carmine Willis (Jim) and Mandy Lomax (Alan); his great grandchildren, Kollen Robertson (Matt), Raegan Willis, Cameron, Anna Kathryn Penley, Hunter, Grant Penley, Sydney and Davis Lomax; his great great grandchild Fisher Carmie Robertson; and his brother Elbert "Eb" Cook. He is also survived by numerous nieces, nephews and cousins.
